Step 1: Create a new form

Navigate to your Google Forms.

Click on the ‘Blank form’ button to start a new form.

Title the form appropriately and offer a helpful description for the employees who will be filling out the form. 

Step 2: Make the form anonymous

The first and most crucial step in creating a supervisor evaluation form is making your Google form anonymous. This encourages honest and uninhibited feedback and provides employees with a safe space to express their thoughts and experiences without fear of repercussions or bias.

When working with Google Forms, there are a few things you can keep in mind to ensure anonymity in forms:

  • Don’t include questions that ask for the respondent's personal information, like name, email address, etc.
  • Make sure your form doesn’t collect emails by default. You can change this in ‘Settings’.
  • Use the option to ‘limit form response’ carefully. Your form remains anonymous, but your respondents may be discouraged from offering candid replies. 

Step 3: Include field for supervisor details

Now, you want to include a field for the supervisor's name who’s being evaluated. 

Change the default question to ‘Supervisor’s name’ and click the button next to the question field to change the question format to ‘Short answer.’ You may want to add fields for collecting other information:

  • Name of the department to which the supervisor belongs
  • Review for the period
  • Review for the project

To add a question, click the ‘Add question’ button in the floating menu. 

Step 4: Create linear scale questions to evaluate abilities

Now, you can start adding the specific criteria on which you want to invite feedback. We’ll be using the linear scale format for this section. Here are a few aspects to consider:

  • Communication Skills
  • Team Management
  • Decision-Making
  • Problem-Solving
  • Employee Support

Now, add a new question to your form and insert your question in the question field. 

Next, choose the ‘Linear scale’ question format.

Mark your labels appropriately. You can also choose to have more or fewer labels for a more nuanced evaluation of a particular skill. Add all your other linear scale questions in this manner. Other questions may include the following:

  • To what extent does the supervisor encourage collaboration and teamwork within the team?
  • How well does the supervisor provide feedback and support employee development?
  • How would you rate the supervisor's ability to make timely and informed decisions?
  • How available and supportive is the supervisor in assisting with employee growth and well-being?

Step 5: Include open-ended questions for more feedback

For an evaluation to be balanced, gathering objective and subjective feedback is ideal. We’ve already added questions to collect objective data. Now, let’s add some open-ended questions to this evaluation form. Here are a few subjective questions to consider:

  • Describe a recent situation where the supervisor effectively communicated expectations or changes within the team.
  • In your opinion, what are the supervisor's greatest strengths as a leader, and how do these strengths positively influence team dynamics and performance?
  • Can you share any suggestions or areas for improvement you believe the supervisor should focus on?

To add these questions, click the ‘Add question’ button and select the ‘Short answer’ or the ‘Paragraph’ format from the options provided by Google Forms.
