How to add a logo to your Google Forms
Step 1: Open up the Google Form
Navigate to Google Forms. Find and click the form where you’d like to add a logo under your recent forms. If you don’t see the form you’d like to work on, you can use the search bar.
Step 2: Open Theme settings
When your form opens, click the “Customize theme” button (color palette icon) in the top right corner. Doing this will open up a side panel where you’ll see all the theme settings.
Step 3: Choose header image
Click the “Choose an image” button under “Header” in the theme settings.
This will open a window where you’ll see three options for selecting a header image: Themes, Upload, and Photos. Themes has Google-provided images in several categories, Upload allows you to upload from your device, and Photos lets you pick from your Google Images.
When you click Upload, a window will open up. Click “Browse” and follow the steps to upload your brand logo to your Google Forms.
When you upload your logo, you can resize and reorient it before it’s applied to your form. Make sure the header is the right size so it doesn’t look warped!
When you click “Done,” your logo will appear at the top of your form as a header.
You can further enhance the appearance of your image by playing with the color settings in the side panel. You don’t need to worry about color schemes clashing, as Google Forms will pull the most flattering colors from your logo’s color scheme and display it in the settings.
With that, you’ve successfully added your logo to your Google Form. There’s one more way to display your logo in your Google Form: you can add it to a question.
Step 4: (optional) Add logo to a question
Let’s add a logo to a question in Google Forms. Go to the question where you want to add the logo. Click the “Image” button on the right side of the question.
You’ll see all the source options available to insert your logo on the top, but we’re going to choose “Upload.” Click Browse and upload your logo.
Now your logo will appear as an image in your question. If you want to change the positioning of the logo, click on the three dots in the left corner.
By doing so, you can re-align your logo any way you like. You can change, remove your logo, or even add a caption.
Click on the preview button on the top right corner of the form to see how your logo appears in front of your respondents in the question.

Once the logo appears in your form, align it centrally. Leave the question field empty.
