How to include a link on a Google Form
Step 1: Create a new Google Form

It’s important to know that the text you input in the options when inserting a multiple-choice or a multiple-choice grid question can’t be hyperlinked.
Step 3: Use the “Insert Link” option
Once you’ve inserted the text you want to hyperlink, select it. Find and click the “Insert link” button in the formatting options that open up below it (it looks like a chain link).
Step 4: Paste the destination link
Clicking the “Hyperlink” button will open up a new window. Enter the text you want to be hyperlinked.
In the next field, insert the link and click “OK.”
